Event Description:
The start of school is just around the corner and so is your community urgent care! Piedmont Urgent Care by WellStreet loves being a part of your school community and provides expanded access to quality care. As an opportunity to meet local community members and introduce our team to your teammates and parents, we are providing FREE Sports Physicals August 1st – August 31st at the Johns Creek location ONLY.
What to Know
Please have your athletes bring any necessary forms required by your school and glasses if they wear them. Minors require a parent or guardian present at time of visit. If the athlete is under the age of 18 & does not have a parent or legal guardian present, Piedmont Urgent Care by WellStreet cannot treat the patient; a parent must be present to discuss previous medical history and make sure follow care/instructions are addressed per the doctor’s recommendation.
